Objective. To conduct a comparative retrospective analysis of the anatomical and functional results surgical treatment proximal humerus fractures AO/OTA 11-B, 11-C using different methods in patients over 50 years old with osteoporosis to justify differential approach. Methods. The study included 102 aged older 11-C. were divided into three groups: I — (16 men, 34 women) whom performed open reduction internal fixation (ORIF) PHILOS plate; II 44 (8 36 ORIF plate 3D polylactide (PLA) porous implants applied; III 8 (2 6 primary reversed total shoulder arthroplasty (RTSA) developed reversible endoprosthesis. evaluated according Constant-Murley Shoulder Score system after 3, 6, 12 months. results. Positive first group obtained 72.0 % (the average months was 78.4 points); second 81.8 (88.0 third 75.0 (82.0 points). A differentiated approach choice is proposed. clinical trial reverse modular endoprosthesis by us titanium parts, manufactured additive technologies, showed positive short-term design features device increase reliability durability proposed Conclusions. RTSA case unstable fracture or metal structure migration low bone mineral density effective intervention that allows achieve satisfactory up 3 years.
